I am in the process of implementing an internal audit. Can someone tell how they report the findings in an internal audit. When a finding needs to be reported is there a form used?

You are very savvy and rightfully protective of these children's rights by 'wondering' if you can give all of this data to non-medical sources.  I would say emphatically NO.

 Having all this crucial indentifying information for these children and that they are associated with psychiatric care to a non-medical source is very skewed both legally and ethically.  I would be highly reticent to release any data that had the clear identity of these children.  The protective laws of children have a greater scope and durability than that of adults.  They are clearly not of the age of consent and this information could be harmful to their success in jobs or other ventures.  A medical need to know or provide care has not been demonstrated.  A relationship between the need to know and the source given information has not been demonstrated.
