Lee Ann, You may want to also add to your "fields" a column that flags whether you have special considerations in play since you may be dealing (assuming by your signature block) with consent for treatment implications since you are dealing with minors. This can be a prime determinant to the extent, if any, on whether you may share information with the parents of the patient as indicated by the specifics of the issue. Just pointing this out as I certainly feel it is an important factor to keep in mind. (P.S. I'm guessing you will get a nice number of replies to your request. A quick question that may help as sometimes templates can be a bit "tricky" based on software and their versions.
What versions of Word or Excel are you using?
Also, if you are using Microsoft Office, you very likely and easily have a nice set of tools available that you can use to supplement your tracking tool in the management of these incidents as they are reviewed and addressed without having to invest in additional software.

I am in search of inexpensive software or a easy to edit/use template for word/excel or access to track issues. Something that would allow me to easily track call/email date caller issue category (hipaa/osha/compliance/documentation/etc) priority status issue description (narrative) plan (narrative) folllow-up (narrative)
I imagine there is a way to set something up in access where it could be saved as a report but I'd also like to be able to drag and drop or save pertinent emails into it.
Anybody have something they like that's user friendly? I'd appreciate it.
