    ​Here's an interesting situation....

    We recently had a patient die in our hospital.  After their death, it became apparent that the information provided at the time of admission was incorrect.  The patient's state issued identification card was presented and that information was used; however now it appears the ID was fake, and the date of birth and possibly even their name was incorrect/fake.  The patient had no insurance and was estranged from family.  The family has supplied a different date of birth.

    So we have a state issued ID that seems to be a fake and we have the word from a member of the patient's estranged family.  Do we change a medical record?  The issuance of a death certificate/burial by the county are being held up.

    Appreciate any thoughts.

    If you accept that the ID is fake then you have a "john/jane doe".  Do you have a policy for this?  If my staff brought this to me, I'd probably say contact the medical examiner's office as they might be the one to identify an individual.
