  • 1.  Amendment Request for Previous EMR system

    My organization recently switched EMRs. While we still have access to the previous EMR system, it is read only.

    I recently received an amendment request for information documented in the previous EMR system.

    I've passed along the amendment request to the clinicians who completed the documentation to determine whether it is a valid request.

    Q:  If the amendment request is valid, how can the amendment process be completed in a EMR system that we cannot add an amendment to?
     - The amendment request will be documented in the new EMR system.

    Paul,

    I have had this scenario before and I just had the new document scanned to the new EMR.  This sometimes happens when a document is corrupt or not interfacing appropriately from other systems.  Assuming your patient demographics carried over, I would just scan amendment to new EMR.

    Makes sense!  I think I was trying to overanalyze this.  Just goes to show that sometimes there are too many trees in the way of seeing the forest!!

    Just  thought and probably not relevant to this instance of the question:

    If you have ever been looking for reasons to argue against keeping records beyond the mandatory retention time this example might help support your position. If you are maintaining the records they are subject to first party right of access and amendment requests and everything else to which the designated record set is subject.

    That's an excellent point! Purging charts, etc. at the appropriate time is so important!
