    De-identified data by removing all of the identifiers listed in the Privacy Rule from a dataset results in the data no longer representing PHI and there is no longer a risk of identification.

    I would say false.  There is still a risk of identification.  The risk lies in the fact that the person can still potentially be re-identified when the de-identified information is combined with publicly available information.

    False. Removing the identifiers means the data is no longer PHI. However, the information may be re-identified if there is a (1) key / code for re-identification or (2) the data is combined with publicly available data or (3) the data is combined with other data another party has.

    I believe the answer is False. The highlighted segment of the question is the rationale for my answer.
