  • 1.  Request for Amendment

    What are your thoughts on a name change, do you consider this an amendment to a patient's PHI and account for it separately as you would if they requested an amendment to a clinical test result or some other clinical amendment?

    It's my opinion that demographic information should not need an amendment.  However, for a name change we would require something official, like a driver's license, that has their new name on it.

    Thank you, we do require identification, scan in the identification/documents that are necessary for the change and note it in the  EHR, so there is a record of it.

    Pamela,

    Not saying others are wrong or right...just adding another perspective.

    For me, I see an amendment as a change/edit/update. Since a person's name is an identifier as well as information that can be used to identify a person and is often obtained through the course of providing treatment, I see that using your amendment of PHI process would apply to a situation here someone wants to amend their PHI as a result of a name change.  Seems pretty straightforward to me.

    I do not see this needing an amendment; however, we have them fill out a request to update patient demographic information form. Goes along with other demographic information. Require some official form in order to change information like Cinda suggested.
