    We are a Continuing Care Community with Independent Living, SNF, HH, Hospice and Assisted Living Services. Our community publishes an annual Resident Handbook that has reference information as well as a listing of resident names and addresses (for those who have not opted out). Previously we have not included our SNF residents in this listing as identifying them by name and address would indicate the are receiving health services. There is some interest in including these individuals. We have a Facility Director policy but is written in relation to a traditional directory one would use in a healthcare facility, not a community handbook that would be distributed to all residents and employees on the campus. I have a concern about allowing this inclusion for our SNF and Assisted Living residents without attaining specific authorization from them for their inclusion, other than the traditional "opt-out" option for a facility directory. Would appreciate your thoughts. Thank you.
